Grade 7-8 Immunization Clinic

Ottawa Public Health will be holding immunization clinics at the school on Monday Nov 23 and Tuesday Nov 24 2015. These vaccines are publicly-funded, and available at no cost to you.

 On Monday November 16th  your child will receive  the vaccine consent form(s) from Ottawa Public Health.  Your child needs to return the signed consent form(s) to the nurse on the day of the clinic in order to receive the vaccines.  

 All students in grade 7, and students born in the year 2003, have received consent forms for hepatitis B and meningococcal disease vaccines. 

 All female students in grade 8, and female born in the year 2002, have received a consent form for human papillomavirus (HPV) vaccine.

 You are encouraged to talk with your child about the vaccines; and ideally both you and your child should sign the consent form.

 On the day of the school clinic, remind your child to:

      • Eat a good breakfast.
      • Bring an extra juice or a snack to school.
      • Wear short sleeves or a loose shirt so that his or her upper arm is exposed easily, without undressing
      • Bring the signed consent forms with them to the school clinic

Students who miss a clinic will be able to receive the vaccine at the next school clinic.
